Notice of Privacy Practices

Your Information. Your Rights. Our Responsibilities.

Effective Date: August 21, 2026

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Our Responsibilities

We are required by law to protect the privacy and security of your protected health information (PHI), give you this Notice, and follow the privacy practices described here. If a breach of unsecured PHI occurs, we will notify you as required by law. Unless the law allows or requires it, we will not use or share your PHI for purposes not described in this Notice without your written authorization. You may revoke an authorization in writing.

How We May Use and Share Your Information

Treatment, Payment, and Practice Operations

We may use or share your information to provide and coordinate dental care, communicate with other treating providers and dental laboratories, verify insurance benefits, submit claims, obtain payment, collect accounts, and operate and improve our practice.

Patient Photographs and Clinical Images

We may take patient profile photographs and clinical images as part of your dental record.

We may use them for identification, documentation, treatment planning, progress evaluation, communication with laboratories or other providers, insurance support, and other appropriate practice purposes.

We will not use identifiable patient photos or other identifying information for advertising, our public website, social media, testimonials, or other promotions without your express permission or written authorization when required by law.

Appointments and Health-Related Communications

We may contact you about appointments, follow-up care, treatment options, benefits, or other health-related services.

When the Law Allows or Requires It

We may use or share information for certain public health, safety, oversight, workers’ compensation, legal, law-enforcement, or research purposes when permitted or required by law. We will also provide information to the U.S. Department of Health and Human Services when required to investigate or determine compliance with HIPAA.

Business Associates

We may share PHI with companies that help us operate the practice, such as billing, information technology, record storage, dental laboratories, or payment-service vendors. When required, they must protect PHI under applicable law and written agreements.

Substance Use Disorder Records

If we receive, create, or maintain substance use disorder records protected by 42 CFR Part 2, we will handle those records under the additional protections required by federal law. Part 2 records generally may not be used or disclosed in civil, criminal, administrative, or legislative proceedings against you unless the law’s consent, court-order, and other legal requirements are met.

Marketing, Fundraising, and Promotional Use

We do not sell, rent, or trade PHI to third parties for their own marketing. We obtain written authorization when HIPAA requires it for marketing, a sale of PHI, or other uses. If we contact you for fundraising as permitted by law, you may opt out of future fundraising communications.

If you voluntarily provide pictures, stories, letters, biographies, thank-you notes, or similar materials, we will not publicly use identifying information from those materials for promotional purposes without your express advance permission or written authorization when required by law.

Your Rights

You have the right to:

  • See or get an electronic or paper copy of your dental record and other health information we maintain about you.
  • Ask us to correct information you believe is wrong or incomplete.
  • Ask us to contact you in a specific way, such as at a particular phone number, email address, or mailing address.
  • Ask us to limit certain uses or disclosures. We do not have to agree to every request.
  • If you pay in full out of pocket for a service, ask us not to tell your health plan about that service for payment or health care operations unless the law requires us to do so.
  • Ask for a list of certain disclosures of your PHI made during the six years before your request, subject to legal exceptions.
  • Get a paper copy of this Notice at any time, even if you agreed to receive it electronically.
  • Have a legally authorized personal representative exercise your privacy rights for you.
  • File a privacy complaint without retaliation.

Your Choices

When applicable, you may tell us whether you want us to share relevant information with family members, close friends, or others involved in your care or payment for your care. If you cannot tell us your preference, we may share information when the law permits and we reasonably believe it is in your best interest.

Changes to This Notice

We may change this Notice and apply the revised terms to all PHI we maintain. The current Notice will be available upon request, at our office, and on our website.
