What to Expect
Our team looks forward to welcoming you to Newpark Dentistry, PC! Our office is located conveniently just off of I-80 at 1441 W Ute Blvd, Suite 200 in Park City, near Kimball Junction.
Your first appointment gives us a chance to get to know you and learn about your dental history, discuss any concerns, and evaluate your overall oral health. Depending on your needs, your visit may include dental X-rays, a comprehensive examination, a periodontal evaluation of your gums and supporting tissue, and a professional cleaning. At the end of your appointment, we’ll review our findings together and discuss any recommended treatment or next steps. Our goal is for you to leave with a clear understanding of your oral health and what to expect moving forward.
New Patient Paperwork
Although we appreciate patients completing New Patient Forms before your appointment, patients are also welcome to complete the forms in office. The form includes your medical history and other information our team will review before your visit.
Bring the completed form to your appointment or fax it securely to our office at (435) 604-0885. Please confirm that you are using the correct fax number before sending paperwork containing medical or insurance information.

Insurance and Payment
We know dental insurance and payment options can sometimes be confusing, and our team is happy to help. We accept most major dental insurance plans and can assist you in understand your estimated benefits and anticipated out-of-pocket costs. Because coverage varies by plan, final benefits and coverage are determined by your insurance provider.
Payment is due at the time of service unless other arrangements have been made. For your convenience, we accept major credit cards and offer secure online payment.
Payment Plans
We’re happy to partner with Care Credit to offer patients an additional payment option. CareCredit may provide more flexibility by allowing qualifying patients to make payments over time.
